The Cruelty Guild

The Cruelty Guild The late afternoon sun filtered through the blinds of Dr. Howy’s modest New York clinic, casting warm, golden stripes across a room filled with the scent of lavender and herbal teas. For over twenty years, this office had been a sanctuary. Here, patients weren’t treated as a collection of symptoms or commercialized labels; they were listened to, supported with holistic care, and treated with radical empathy. But today, the air felt heavy. Dr. Howy sat at his desk, staring at a formal envelope bearing the seal of the State Psychiatric Oversight Committee. It was the third "administrative review" notice he had received since publishing his whistleblower reports online. In that report, he had pulled back the curtain on the institutional overreach of the local psychiatric network—exposing how peaceful, vulnerable individuals were being aggressively over-medicated and stripped of their autonomy just to feed corporate-backed bottom lines. The doorknob turned, and Dr. Howy’s assistant poked her head in, her face pale. "Dr. Howy? There are two men here to see you. They say they're from the board." Before he could answer, the door was pushed open. Two figures stepped into the warm room, instantly draining it of its comfort. They wore sharp, dark suits, and their expressions carried the clinical detachment of men who viewed human emotion as a variable to be managed. Leading them was Dr. Vance, the chief administrator of the regional psychiatric guild. "Dr. Howy," Vance said, his voice flat and perfectly practiced. "We’re here to conduct an immediate, unannounced audit of your practice. There have been... concerns raised about your methods." Dr. Howy stood up, his posture calm but firm. "My patients are thriving, Dr. Vance. They are finding genuine healing through nutrition, lifestyle adjustments, and informed consent. If you’re here about the patient files, you know I protect their privacy fiercely." Vance smiled, a cold, mechanical movement of the lips that didn’t reach his eyes. "We aren't interested in your smoothies and vitamins, Howy. We are interested in your public defiance. Your recent articles accuse our profession of manufacturing cruelty. That is a dangerous, non-compliant narrative." "I spoke the truth," Howy said softly, looking Vance in the eye. "I watched your department label normal human grief as a disease, strap a terrified young man to a bed, and punish him for the very distress your system aggravated. That isn't care. It’s a closed guild protecting its power." The second administrator stepped forward, opening a leather portfolio. "Your opinions are irrelevant. Because you have publicly questioned standard psychiatric protocols, we have reason to believe your own cognitive faculties are compromised. Under Section 4, we are initiating an emergency evaluation of your mental fitness to practice medicine." The irony was suffocating. The very weapon Dr. Howy had blown the whistle on—using labels to marginalize and silence anyone who didn't fall into line—was now being pointed directly at him. They weren't just trying to revoke his license; they were trying to turn him into a patient, to wrap him in the same suffocating system he fought against. "You want to call me crazy because I believe in human compassion?" Howy asked, a hint of a sad smile appearing on his face. "We call it 'unprofessional non-compliance,'" Vance corrected coldly. "You have twenty-four hours to retract the articles and hand over your clinical records, or we will proceed with the emergency order. Think of your reputation, Howy." They turned and walked out, leaving the door wide open. Dr. Howy stood alone in the quiet office as the sun began to dip below the horizon. The threat was real, and the machinery of the guild was massive, heavily armed with regulations they wrote themselves to protect their own coldness. He looked down at his desk, where a fresh stack of holistic wellness guides sat waiting for his patients tomorrow. He felt the fear, but beneath it, an unshakeable sense of purpose. They wanted him to hide, to cower, and to disdain the very people he spent his life protecting. Dr. Howy sat back down, opened his laptop, and began to write his next article. If the guild wanted a fight, they would find that compassion wasn't a weakness—it was something the monsters could never truly lock away. Speculative Fiction by Dr Harold Mandel
